DSO Letter Request Form

Please fill out this form in order for HIU to create a DSO letter for you. Bring this letter to your appointment at the Social Security office. A copy or printed version of the DSO's version of the signature will not suffice. Please submit your information exactly as it appears on your ID (usually a passport), even if you spell your name differently for other purposes.
